Key Responsibilities\n

• Develop and implement personalized treatment plans that integrate both psychiatric and medical care to address patients' complex needs.

• Facilitate individual and group therapy sessions using evidence-based techniques tailored to each patient's specific situation.

• Collaborate with other healthcare professionals to ensure coordinated treatment and care, particularly for patients with complex medical histories.

• Provide crisis intervention during mental health emergencies, employing effective communication and de-escalation techniques.

• Educate patients on the interplay between mental health and physical health, empowering them with the knowledge to manage their conditions.

• Maintain comprehensive and confidential documentation in accordance with legal and ethical standards, utilizing electronic health record systems.

• Engage actively in team meetings, case conferences, and collaborative care planning to enhance treatment outcomes.

• Stay informed of the latest research and developments in integrated care to continuously improve patient care practices.

Patient Population / Client Focus\n

You will primarily work with individuals facing multifaceted mental health challenges, including those with co-occurring medical conditions such as autoimmune disorders, chronic pain, or substance use disorders. Your role will be centered on understanding and addressing the unique challenges these patients face, promoting their overall well-being in a nurturing environment.

Qualifications\n

• Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LCSW, LPC, or LMFT in New Jersey.
\n
• Proven experience in addressing the mental health needs of individuals with overlapping medical conditions.
\n
• Strong interpersonal skills with the ability to build rapport and foster a trusting therapeutic relationship with patients.

• Proficiency in electronic health record systems and other relevant software.

• A commitment to maintaining ethical standards and patient confidentiality.

Preferred Experience\n

Compensation and Benefits\n
