Key Responsibilities\n

• Conduct thorough psychosocial assessments by engaging in interviews, performing direct observations, and reviewing medical records, to identify factors affecting patients' emotional and environmental well-being.

• Deliver empathetic counseling services to patients and their families dealing with grief, adjustment to illness, and emotional distress stemming from advanced health conditions.

• Collaborate with an interdisciplinary team of healthcare professionals, including physicians, nurses, and occupational therapy specialists, to devise and implement personalized care plans based on each individual's needs.

• Guide patients and their families in locating and utilizing community resources, such as support groups, financial assistance services, and end-of-life planning tools.

• Facilitate education sessions for patients and families about available community resources, enhancing their understanding of post-rehabilitation care options and opportunities.

• Participate actively in discharge planning to ensure seamless transitions for patients approaching the end of their life journey, addressing both emotional and logistical aspects.

Patient Population / Client Focus\n

This position mainly serves individuals in hospice care, including those experiencing terminal illnesses or severe health challenges. Our clients often present complex emotional and psychological needs, which require the LCSW to employ a deep understanding of trauma-informed care and grief counseling.

Qualifications\n

• Hold a current, valid LCSW license in California.

• Possess a Bachelor’s degree in Social Work or a related field.

• Demonstrate a minimum of one year of relevant experience in providing clinical social work, ideally in a hospice or rehabilitation setting.
