Key Responsibilities

• Conduct thorough evaluations of patients, reviewing pertinent medical histories and performing assessments to determine effective rehabilitation plans.
• Communicate evaluation results and treatment plans clearly to patients and their families, ensuring their active participation in setting recovery goals.
• Implement individualized treatment protocols informed by evaluation findings and patient presentations, adapting approaches based on ongoing assessments and recovery progress.
• Collaborate with multidisciplinary teams and consult with other healthcare professionals to address changing patient conditions, ensuring timely modifications to treatment plans as necessary.
• Document patient care activities with precision, including initial evaluations, daily notes, and discharge plans that clearly reflect the patient's journey and treatment goals.
• Manage time efficiently to meet productivity targets while maintaining a full schedule of patient appointments and meetings.
• Engage in hospital and community outreach initiatives, promoting awareness of our services and increasing access to care for the community.

Patient Population / Client Focus

Qualifications
