Key Responsibilities

• Direct the delivery of physical therapy treatment plans aimed at improving muscle function, restoring mobility, and preventing disabilities in patients.
• Conduct thorough evaluations of patient physical abilities using diagnostic tests to assess muscles, nerves, joints, and functional capabilities.
• Collaborate with physical therapy assistants to ensure effective implementation of care plans.
• Administer manual therapeutic exercises, along with motivating and instructing patients in self-care and functional activities.
• Develop and modify personalized rehabilitation strategies based on individual patient assessments and progress.
• Monitor and document changes in patient condition, sharing updates with physicians and case managers to optimize treatment plans.
• Engage in interdisciplinary team meetings to provide insights and updates on patient progress, contributing to comprehensive care management.
• Prepare and maintain accurate clinical records including initial evaluations, treatment plans, progress notes, and discharge summaries.
• Instruct patients and their families on the use of physical therapy equipment and best practices for managing their treatment plans at home.
• Participate in continuing education initiatives to stay current with advancements in physical therapy practices and technologies.
