Key Responsibilities\n

• Act as a liaison between clients, their families, and outpatient providers, ensuring everyone is aligned on care plans and progress.

• Engage with insurance companies to facilitate pre-certification processes and manage utilization effectively to provide the best care.

• Participate actively in discharge and aftercare planning, ensuring continuity of care and appropriate resource allocation for each client.

• Provide therapeutic meal support by modeling healthy relationships with food, integral for clients dealing with eating disorders or food-related issues.

• Deliver safe, empathetic, and affirming care tailored to each client's unique circumstances, with a strong understanding of intersecting identities.

Patient Population / Client Focus\n

The LCSW will primarily engage with a diverse client base, including older adults facing various life transitions, mental health challenges, and addiction issues.
