Key Responsibilities\n

• Serve as a primary liaison between clients’ families and outpatient healthcare providers, fostering open lines of communication to discuss treatment plans and progress.

• Interact with insurance companies to navigate pre-certification processes and facilitate utilization management in a timely manner.
\n
• Participate in discharge and aftercare planning, developing strategies that facilitate a seamless transition from hospice care back to home or other living arrangements when appropriate.

• Conduct therapeutic meal support sessions, helping clients liberate themselves from dysfunctional food relationships and cultivate positive habits.
\n
• Deliver empathetic, sensitive counseling tailored to each client's unique circumstances, recognizing their intersecting identities such as age, gender, culture, and personal experiences.

Patient Population / Client Focus\n

In this role, you will work primarily with older adults, many of whom are navigating complex emotional and psychological challenges associated with end-of-life care. Your expertise will play a pivotal role in ensuring that clients and their families feel supported, understood, and respected throughout their journey.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Must possess a valid LCSW license in the state of Maryland.

• Demonstrated experience working with older adults, especially in palliative or end-of-life care settings.

Compensation and Benefits\n
