Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients’ mental health needs, strengths, and available resources, ensuring a thorough understanding of each individual’s circumstances.

• Provide culturally sensitive individual, family, and group therapy utilizing a variety of evidence-based treatment modalities, including cognitive-behavioral, solution-focused, and trauma-informed therapies.

• Develop individualized treatment plans that include specific goals and targeted interventions, regularly updating them based on client progress and feedback.

• Collaborate closely with psychiatrists, psychologists, and a multidisciplinary team to coordinate care and ensure comprehensive support for clients.

• Engage in crisis intervention as needed, performing risk assessments and developing safety plans to protect clients in vulnerable situations.

• Facilitate group therapy sessions, psychoeducational programs, and support groups for clients and their families, addressing topics related to grief and mental health.

• Maintain accurate documentation and case files, ensuring compliance with ethical and legal standards within the mental health profession.

As a Licensed Clinical Social Worker in our hospice facility, you will work primarily with terminally ill patients and their families.
