Key Responsibilities\n

• Conduct thorough ongoing assessments for patients to tailor personalized treatment plans that address individual needs and goals.

• Regularly communicate with supervisors, healthcare professionals, family members, and other relevant stakeholders to provide updates on patient progress and adjust treatment strategies as necessary.

• Prepare and maintain a tentative schedule for appointments, home visits, and observations that monitors the effectiveness of the treatment provided.
\n
• Coordinate and execute home visits, enabling comprehensive in-home observations to evaluate patient environments and support systems.
\n
• Ensure effective implementation of program plans and actively participate in interdisciplinary team meetings to discuss patient needs and progress.
\n
• Document monthly and quarterly progress notes that clearly describe patient advancements and any adjustments made in treatment approaches.
\n
• Lead training sessions to ensure all team members are proficient in program implementation and documentation processes.
\n
• Conduct follow-ups, including interviews and additional observations, to ensure continuous support for the clients’ evolving needs.
\n
• Maintain accurate and organized patient records to facilitate easy access for authorized personnel.
\n
• Act as a liaison with the public and mental health community, advocating for policy development that aligns with organizational goals.

• Assist in creating and refining training materials to enhance the effectiveness of programs offered to individuals.

Patient Population / Client Focus\n

The LPC will primarily work with a diverse clientele that includes adults and youth facing a variety of mental health challenges. This population may include those dealing with family dynamics, trauma, substance misuse, and behavioral health issues. Our focus is on providing empathetic and personalized care to support the unique journeys of each client.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• A valid and unrestricted license as a Professional Counselor in the state of Kansas is mandatory.

• A minimum of two years of counseling experience is required, particularly in clinical settings.

• Strong understanding and adherence to legal and ethical standards in mental health practice.

• Excellent interpersonal and communication skills, enabling the establishment of rapport with clients from various backgrounds.

• Demonstrated ability to collaborate effectively within a healthcare team.

• Proficiency in Electronic Medical Record (EMR) software and familiarity with relevant technologies is essential.

Preferred Experience\n

• Specialization in family therapy, couples therapy, trauma, or substance misuse is highly preferred.

Compensation and Benefits\n
