Licensed Mental Health Counselor - Geriatric Behavioral Health

Senior Care Therapy seeks a Licensed Mental Health Counselor to provide compassionate behavioral health services to seniors in long-term care, assisted living, and community settings. In this role, you will complete comprehensive assessments, develop person-centered treatment plans, and deliver individual and group psychotherapy both onsite and via telehealth. You’ll collaborate closely with interdisciplinary teams and facility partners to address mood, anxiety, and adjustment issues, improve residents’ quality of life, and support staff education. Responsibilities Deliver individual and group psychotherapy to seniors in long-term care, assisted living, and community settings. Conduct comprehensive psychosocial and diagnostic assessments using DSM-5. Develop, implement, and update person-centered treatment plans. Coordinate care and collaborate with facility staff, families, and medical providers. Document services in EHR in accordance with regulatory and ethical standards. Provide telehealth counseling and follow-up as appropriate. Participate in interdisciplinary team meetings and case conferences. Educate staff and families on residents’ behavioral health needs and interventions. Maintain licensure, continuing education, and adherence to best-practice guidelines.
