Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, contributing expertise as part of a multidisciplinary team.

• Conduct thorough patient intakes, focusing on diagnostic evaluations and functional assessments, to create tailored treatment plans in alignment with each patient’s unique needs.

• Provide brief, focused interventions for patients requiring mental health services, employing a range of therapeutic techniques including cognitive behavioral therapy and motivational interviewing.

• Advise primary care providers on the appropriateness of mental health interventions, facilitating timely referrals to specialty services when necessary.

• Monitor patient progress and initiate follow-ups to assess treatment effectiveness, making necessary adjustments to care plans as warranted.
\n
• Develop and implement relapse prevention strategies for at-risk patients to promote sustained well-being.
\n
• Coordinate health services for patients related to behavioral health care, fostering connections with other providers both within and outside the primary care model.

• Keep accurate and up-to-date clinical documentation within treatment charts, ensuring compliance with agency and regulatory standards.

• Provide consultation to clinic management and participate in the development of strategized behavioral health services that enhance patient outcomes.

• Assist patients in navigating and complying with medical treatments initiated by their primary care providers, offering tools to manage side effects effectively.

• Maintain high ethical standards, adhering to the Code of Ethical Conduct for Clinical Professionals.

Patient Population / Client Focus\n

Your work will involve engaging with a diverse range of patients, including those facing mental health challenges stemming from various physical and emotional disorders. The counselor will provide essential support focused on improving outcomes by equipping patients with coping strategies and self-management skills critical to their recovery process. Additionally, you will work with families, guiding them through mental health issues and offering resources that assist in holistic healing.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling.

• Current and valid LPC (Licensed Professional Counselor) License in Texas.

• Comprehensive knowledge of mental health assessment techniques and treatment planning, along with a thorough understanding of developmental psychopathology.

• Familiarity with DSM-IV diagnoses and behavioral therapy methods.
\n
• Strong verbal communication skills to effectively engage with a bilingual client base; proficiency in English and Spanish is preferred but not required.

• Excellent organizational skills enabling the documentation of clinical records and preparation of reports.

Preferred Experience\n

• Experience working in a primary care setting or a Federally Qualified Health Center (FQHC).

• History of developing successful treatment plans and interventions for complex patient populations.

Compensation and Benefits\n

This position offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications. In addition, we provide a sign-on bonus as an incentive for joining our team.
