Key Responsibilities\n

• Engage proactively with clients to prevent crises and enhance community stabilization, using various therapeutic modalities tailored to individual needs.

• Conduct thorough assessments and maintain crisis alerts to effectively coordinate with local county crisis services when necessary.

• Facilitate transitions for clients requiring hospital or inpatient services, working closely with medical professionals to ensure smooth discharge planning and continuity of care.

• Utilize a variety of telehealth tools and therapy platforms to document client progress and treatment plans efficiently.

Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from $100,000 to $105,000 annually, based on a candidate’s caseload and experience.
