Key Responsibilities\n

• Document transitions and facilitate patient transfers to and from hospital/inpatient services, working collaboratively with healthcare teams to ensure smooth discharges.

• Utilize crisis de-escalation techniques to support individuals facing emotional distress in a safe and constructive manner.

• Conduct thorough risk assessments and develop tailored safety plans to protect clients in vulnerable situations.

• Guarantee timely documentation on same-day assessments and interventions for clients at increased risk of crisis interactions, adhering to organizational standards for quality care.

Compensation and Benefits\n
