Key Responsibilities\n

• Engage in the development and organization of program services that cater to our patients' mental health needs.

• Maintain open lines of communication with physicians to provide timely updates regarding patient progress, needs, and any potential issues that may arise.

• Document client interactions meticulously, ensuring that clinical and progress notes are recorded in compliance with departmental guidelines and timelines.

• Collaborate closely with a multidisciplinary healthcare team to create effective treatment plans, including aftercare and discharge strategies tailored to each patient’s psychosocial assessment.

• Provide counseling sessions and crisis intervention for patients experiencing emotional and behavioral challenges, tailoring approaches to meet individual needs.

• Consult with families and support systems to facilitate comprehensive treatment and discharge planning, fostering a supportive environment for patient recovery.

Patient Population / Client Focus\n

In this role, you will primarily focus on high-risk patients, including individuals in detox treatment who are dealing with substance use disorders or severe mental health conditions. Qualifications\n

• A Master’s degree in Social Work (MSW) or related field is required.

• Must possess active state licensure as a Licensed Independent Clinical Social Worker (LICSW).
