Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand the emotional and social needs of patients and their families, emphasizing support during end-of-life transitions.

• Develop and monitor personalized treatment plans that reflect each patient's unique circumstances, preferences, and goals.

• Offer crisis intervention and ongoing support, particularly during acute emotional distress, while conducting risk assessments and safety planning.
