Work Environment

The selected candidate will work onsite at our rehabilitation facility located in Bradenton, Florida. Key Responsibilities

• Conduct thorough evaluations including a review of patient history, performance assessments, and documentation of findings to create individualized treatment plans.
• Communicate evaluation results and treatment recommendations clearly to patients and their families, ensuring they are engaged in setting realistic goals for recovery.
• Implement evidence-based treatment protocols based on evaluation results, adapting methodologies in response to the patient's progress and the evolving healthcare landscape.
• Collaborate with multidisciplinary healthcare professionals as required, continuously monitoring and documenting changes in patient conditions that may require alterations to their care plans.
• Manage discharge planning and ensure appropriate follow-up care by documenting all patient interactions, ensuring clarity and continuity of care.

Patient Population / Client Focus

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.
• Current state licensure as a Physical Therapist.
• Basic Life Support (BLS) certification through the American Heart Association for Healthcare Providers is mandatory.

Preferred Experience
