Work Environment

This position is primarily onsite, as you will deliver therapy services directly within patients' homes. This unique setting allows for personalized treatment aligned with each patient's daily routines and environments, fostering a more effective rehabilitation process. You will collaborate closely with a multidisciplinary team of healthcare providers to ensure comprehensive support for our clients.

Key Responsibilities

• Conduct thorough evaluations of patients by reviewing their medical history and performing detailed physical assessments to identify their specific needs and establish personalized treatment plans.
• Effectively communicate the evaluation findings and treatment plan to patients and their families, ensuring they are integral to the goal-setting process.
• Utilize evidence-based practices to implement tailored treatment protocols that adapt to each patient's unique presentation and progress.
• Engage in interdisciplinary consultations whenever necessary to optimize patient care, documenting any significant changes in a patient's condition that may necessitate a modification of their therapy plan.
• Manage discharge planning and follow-up care, ensuring all documentation related to patient care is completed accurately and in a timely manner, including evaluations, treatment summaries, and progress notes.

Patient Population / Client Focus

In this role, you will primarily work with a diverse patient population ranging from post-surgical clients to those recovering from chronic pain conditions or injuries. Your focus will be on delivering care that is sensitive to the individual circumstances and environments of each patient, which often leads to better treatment outcomes while helping them regain their independence and quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• Current state licensure for Physical Therapy in Florida or eligibility to obtain.
• Strong communication skills with an emphasis on patient education and engagement.
• The ability to work independently and manage a flexible schedule that accommodates patient needs.

Preferred Experience

Compensation and Benefits

This position offers a competitive salary ranging from USD 100,000 to 120,000 per year, commensurate with experience and qualifications.
