In this role, you will contribute significantly to the health and welfare of children with disabilities and developmental delays in a hospice center setting. Key Responsibilities\n

As a Licensed Clinical Social Worker, you will engage in a variety of essential functions that contribute to our patients' well-being:

• Conduct detailed evaluations and assessments to identify the needs of children enrolled in our programs.
\n
• Participate in the development and implementation of individualized Family Service Plans (IFSP), incorporating inputs from families and other team members.
\n
• Monitor child progress and service outcomes, adjusting interventions as necessary to promote optimal developmental growth.
\n
• Establish trusting relationships with children and families to foster a supportive environment for learning and development.
\n
• Provide ongoing support and resources to families, equipping them with skills needed to enhance their child's development.
\n
• Utilize evidence-based practices and the Natural Learning Environment framework to inform your approach.
\n
• Ensure compliance with all relevant Federal and State regulations and uphold the standards of care within our organization.

Patient Population / Client Focus\n

Your work will primarily involve supporting infants and toddlers exhibiting developmental delays or disabilities. This critical early intervention aligns with our mission to enhance physical, cognitive, social/emotional, and communication skills for children during these formative years. You'll collaborate closely with families, helping them navigate support systems and ensuring access to vital resources, thereby directly impacting their child's trajectory toward independence and success.

Qualifications\n

The ideal candidate will have the following qualifications:

• A Master's degree or Doctorate in Social Work.

• Current LCSW licensure issued by the California Department of Public Health.

• A valid California driver's license.

• Excellent organizational skills, strong time management capabilities, and superb communication abilities.

Preferred Experience\n

We prefer candidates with at least one year of experience specifically in early intervention with children, enhancing their skill set in assessing developmental needs at this critical stage. Compensation and Benefits\n
