Work Environment\n

This position is remote, allowing for flexibility while providing much-needed services from the comfort of your home. As a member of our hospice center-focused team, you will engage with clients through virtual platforms, conduct assessments, and develop and implement individualized service plans. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify children's needs and strengths, facilitating the development of tailored service plans.

• Collaborate with families and team members to create and implement Individualized Family Service Plans (IFSPs), ensuring progress monitoring and support.
\n
• Provide evidence-based early intervention strategies to families, incorporating Natural Learning Environment Practices to optimize child development.
\n
• Educate families on effective practices and resources to support their children’s development during critical early years.
\n
• Utilize a coaching model to engage families in natural settings, ensuring that therapeutic practices are beneficial in everyday contexts.
\n
• Maintain compliance with federal and state regulations while safeguarding the health and safety of clients and staff.

Patient Population / Client Focus\n

Your primary focus will be on assisting infants and toddlers with developmental delays or disabilities. This vital work entails not just direct engagement with children, but also close collaboration with families to ensure a holistic approach to care and connection. By helping families understand their child's developmental journey, you play a crucial role in their children’s growth and success.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.
\n
• Current licensure as a Licensed Clinical Social Worker (LCSW) as mandated by the Department of Public Health.
\n
• Minimum of one year of experience working directly with children in an early intervention setting.
\n
• Demonstrated ability to integrate various learning styles and adapt treatment plans to meet individual client needs.

• A valid driver's license and the ability to travel, as needed.
\n

Compensation and Benefits\n
