Key Responsibilities\n

• Conduct thorough diagnoses and treatment plans tailored to each patient's unique mental, emotional, and behavioral challenges, continually adjusting care strategies as their status evolves.

• Facilitate referrals to appropriate external services such as community support groups, treatment programs, or specialized providers to enhance patients' overall well-being.

• Collaborate with a multidisciplinary team to develop effective discharge plans, ensuring a seamless transition to lower levels of care or community resources post-treatment.

• Adhere to all relevant federal, state, and clinical facility standards and maintain compliance with the Mental Health Practice Act.

• Effectively prioritize workload by assessing the needs of new, current, and transitioning patients to provide timely and effective interventions.

• Offer social work consultations, psychosocial assessments, and crisis interventions when necessary, particularly in complex cases involving bereavement or trauma.

• Communicate and coordinate efforts with various patient care teams and external agencies to resolve delays and ensure optimal treatment outcomes.

• Act as an advocate for patients and their families, ensuring their voices are heard within multidisciplinary care frameworks.

Qualifications\n
