Center Point Healthcare Solution Management Practice is actively seeking a dedicated and compassionate Adult-Gerontology Nurse Practitioner to join our team in Oyster Bay, New York. This full-time, onsite position primarily involves providing excellent primary care to adults on Medicare. Our Nurse Practitioners operate independently while collaborating with a diverse healthcare team to deliver patient-centered care aimed at improving health outcomes.

Work Environment

As a Nurse Practitioner, you will work alongside physicians, physician assistants, nursing staff, medical scribes, and care management professionals. The position requires a proactive and engaged practitioner who is comfortable in both clinical settings and conducting home visits when needed.

Key Responsibilities

As an Adult-Gerontology Nurse Practitioner, your core responsibilities will include:

• Providing exceptional primary care to patients, including new patient assessments and follow-up consultations.
• Managing office visits and addressing both routine and acute health concerns through thorough examinations.
• Administering annual wellness visits and health risk assessments, requiring holistic evaluation and meticulous documentation to improve patient health records.
• Coordinating care with various healthcare providers, specialists, testing facilities, and support agencies to ensure comprehensive patient care.
• Leading population health initiatives, including ensuring that all eligible patients receive essential preventative screenings such as biannual breast cancer screenings for females.
• Participating in triaging patient concerns over the phone and engaging in proactive outreach for follow-up care.
• Educating patients about their health conditions, assisting them in understanding their treatment plans, and fostering proactive healthcare management.
• Engaging in community outreach and promotional activities to raise awareness about our practice and services.
• Conducting in-home visits as necessary to provide continuity of care and address patient concerns in a comfortable setting.
• Reporting to the Medical Director while collaborating closely with operational leadership on various initiatives and quality improvement projects.

Patient Population / Client Focus

Our primary focus at Center Point Healthcare is on adults, particularly those over 65 years old, who often require more comprehensive and individualized care plans. Qualifications

To be considered for the role of Adult-Gerontology Nurse Practitioner, candidates must possess:

• A Master of Science in Nursing or Doctor of Nursing Practice degree, or equivalent graduate program qualification.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• An active, non-probationary Nurse Practitioner license in the state of New York.
• A current DEA license for prescribing medications.
• Experience in a primary care setting, particularly in internal medicine, geriatrics, or family medicine.
• A thorough understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and conducting Health Risk Assessments (HRAs).
• A passion for teamwork and an eagerness to collaborate across multidisciplinary teams.

Preferred Experience

While not mandatory, the following experiences would enhance your candidacy:

• Prior experience in concierge medicine or house call practices.
• Familiarity with electronic health record systems and telehealth platforms.

Compensation and Benefits

We offer a competitive salary ranging from USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
