Work Environment

This role is based in an onsite home care setting, incorporating hands-on rehabilitation services for patients recovering from orthopedic surgeries, injuries, or chronic conditions. Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing pertinent medical history and performing specific assessments to develop tailored treatment plans.

• Communicate evaluation findings and strategies clearly to patients and their families, involving them in setting realistic rehabilitation goals.

• Implement evidence-based treatment protocols based on evaluation results and patient presentation, adapting to changes in patient conditions promptly.

• Engage in interdisciplinary consultations and recognize when adjustments to care plans are necessary, ensuring seamless communication within the healthcare team.

• Facilitate effective discharge planning and follow-up care, maintaining accurate and timely documentation of patient encounters and progress reports.

• Meet productivity expectations while demonstrating excellent time management skills to maintain an organized schedule and actively participate in marketing initiatives.

• Contribute toward departmental goals, ensuring compliance with all governmental and accreditation regulations.

• Pursue ongoing professional development, serving as a clinical instructor for students and aiding in their training.

Patient Population / Client Focus

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program is required.

• A valid state license for Physical Therapy is required in accordance with local regulations.

• Current American Heart Association BLS for Healthcare Providers CPR certification.
