Key Responsibilities\n

• Conduct comprehensive assessments of patients to evaluate their mental health status and needs, ensuring that personalized treatment plans are created based on ongoing evaluations.

• Maintain open lines of communication with supervisors, family members, and other healthcare professionals regarding each patient’s progress and to collaboratively enhance their treatment journey.

• Develop and provide a tentative schedule of appointments, home visits, and necessary observations to monitor patient progress effectively.

• Plan and coordinate in-home visits that involve chart reviews, staff interviews, and data analysis, ensuring that all interventions are tailored to individual needs.

• Ensure that implementation of program plans is executed effectively, adapting strategies as necessary to meet evolving patient requirements.

• Actively participate in Interdisciplinary Team Meetings, contributing valuable insights and collaboration to improve patient care.
\n
• Document monthly and quarterly progress notes that adhere to best practices, ensuring data is accessible for ongoing evaluations of program effectiveness.

• Organize training sessions for staff and team members, fostering competency in implementing program plans and utilizing documentation methods.
\n
• Conduct follow-up interviews and home visits to sustain support and adaptation of services as per individual needs.

• Manage records meticulously, ensuring that relevant information is easily accessible to stakeholders.

• Serve as a liaison with both the public and mental health community to bolster developmental goals and promote mental health education.

• Contribute to the development of training materials aimed at enhancing program delivery and patient outcomes.

Patient Population / Client Focus\n

You will work with a diverse array of clients, including individuals, couples, families, and youth, who may be facing challenges such as trauma, substance misuse, and other mental health issues. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted license as a Professional Counselor (LPC) in the state of Pennsylvania.
