The job primarily involves virtual interactions, though occasional on-site meetings may take place at our serene hospice center located in Edison, New Jersey. Key Responsibilities\n

• Assessing the mental health status of patients, when necessary diagnosing based on standardized tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM), and evaluating treatment effectiveness.

• Creating individualized treatment plans that reflect the cultural diversity and specific needs of the patient population, while utilizing evidence-based practices to support targeted mental health outcomes.

• Regularly monitoring and documenting patient progress, including interventions and strategies employed to ensure effectiveness and adjusting plans as necessary.

• Collaborating with patients and interdisciplinary teams to establish comprehensive care plans that align with the patients' needs and goals.

• Coordinating referrals and serving as a liaison between patients and other mental health professionals, ensuring effective communication and continuity of care.

• Providing education for patients and their families regarding mental health care, treatment options, and resources available in the community.

• Mentoring students, interns, and other healthcare staff, enhancing the team's overall competency and encouraging a culture of shared learning.

The salary range for this position is competitive, falling between USD 90,000 and 105,000 annually.
