Key Responsibilities\n

• Conduct thorough assessments of patients’ psychosocial needs to formulate effective treatment plans and referral recommendations, ensuring that care is tailored to individual circumstances.

• Provide case management by assisting patients in obtaining essential entitlements and completing necessary forms for supplemental insurance and benefits, thereby promoting self-sufficiency and reducing reliance on emergency assistance.

• Facilitate access to prescription cost assistance through coordinating with pharmaceutical company indigent programs and identifying valuable community resources.
\n
• Act as a dedicated advocate for patients while interacting with community agencies, ensuring their needs are prioritized and addressed.

• Maintain comprehensive, accurate patient records and prepare detailed documentation, including reports that contribute to the overall understanding of patient progress.

• Coordinate transportation assistance as needed, ensuring patients have access to the services and resources they require.

• Participate in clinical and team meetings, contributing to case conferences and collaborating with colleagues to refine treatment approaches and strategies.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s degree in a relevant field such as Social Work, Psychology, Marriage and Family Therapy, or Counseling.

• Holders of one of the following valid licenses: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• Minimum of two (2) years of professional experience in the mental health field, demonstrating a strong understanding of counseling methodologies and patient dynamics.

• Experience working in a telehealth setting, benefiting from familiarity with remote counseling tools and techniques.

• Proficiency in cultural competence, particularly in working with diverse communities in urban settings like San Francisco.

Compensation and Benefits\n
