Key Responsibilities\n

As a Licensed Professional Counselor at Crossbend, your responsibilities will include:

• Assisting primary care providers in identifying, treating, and managing mental health and psychosocial issues, functioning as an integral member of the primary care team.

• Conducting comprehensive patient intakes focused on diagnostic and functional evaluations, making informed recommendations concerning treatment goals and plans.

• Providing brief, focused therapeutic interventions for patients requiring mental health services, ensuring timely access to care.
\n
• Evaluating the continuum of care, advising primary care providers on whether patients should remain in primary care settings or be referred for specialized mental health services.
\n
• Monitoring patient progress and coordinating health services related to behavioral health, including liaising with external treatment providers with patient consent.
\n
• Documenting patient information meticulously in treatment records, ensuring compliance with ethical standards and regulatory requirements.
\n
• Developing relapse prevention plans and assisting at-risk patients in maintaining stability and well-being.
\n
• Engaging in proactive strategies to help patients adhere to necessary medical treatments and cope with medication side effects.

• Participating in consultations with clinic management and team members to review outcomes and enhance program effectiveness.

• Carrying out additional duties as assigned by leadership to support the overall operation of the center.

Preferred Experience\n

Compensation and Benefits\n
