Key Responsibilities\n

• Coordinate hospital and inpatient service transitions for clients, ensuring effective communication with healthcare providers to facilitate smooth discharges and program transfers.

• Utilize advanced crisis de-escalation techniques to support clients, particularly during times of significant distress.
\n
• Conduct and document thorough risk assessments and develop safety plans tailored to individual needs.
\n
• Maintain precise and timely documentation for all clients, ensuring same-day updates for those at an increased risk of crisis intervention.

• Complete critical incident reporting forms within 24 hours when required to ensure compliance and client safety.
\n
• Be on-call for after-hours support on a rotational basis, providing care coordination assistance as needed.
\n
• Engage proactively with clients to prevent the need for crisis services and support them in achieving their personal goals.

• Collaborate with county crisis services to manage and maintain crisis alerts for participants.
\n
• Participate in team meetings and contribute to program improvement initiatives.
