Key Responsibilities\n

• Collaborate with the Medical Director and team to coordinate and streamline mental health treatment plans both within the clinic and in collaboration with outside facilities.

• Provide targeted mental health in-service training sessions for medical, psychiatric, and support staff to enhance their understanding and responsiveness to mental health needs during patient interactions.

• Proactively intervene during mental health crises within the clinic, demonstrating acute assessment skills and de-escalation techniques to ensure patient safety.

• Participate in commitment hearings as necessary, advocating for patient needs based on clinical assessments and treatment goals.
\n
• Develop individualized treatment plans tailored to the needs of each patient, setting measurable goals and appropriate interventions.
\n
• Maintain accurate and detailed documentation of all patient encounters in compliance with HIPAA regulations, ensuring the confidentiality of medical records.

• Verify patients' mental health histories and psychotropic medication use to inform current treatment strategies and interventions.

• Prepare regular reports summarizing treatment progress and outcomes, identifying areas for improvement.

• Coordinate with nursing and mental health staff to facilitate post-release referrals and ensure continuity of care.
