Complete comprehensive admission assessments to evaluate patients' physical, emotional, psychosocial, and environmental needs
Develop and initiate individualized plans of care in collaboration with physicians and the interdisciplinary care team
Perform initial nursing evaluations and ongoing reassessments as needed
Accurately document patient assessments, clinical findings, and care plans in the electronic medical record
Communicate changes in patient condition promptly with physicians, Clinical Team Managers, and other members of the care team
Ensure compliance with HIPAA regulations, company policies, and clinical standards
Promote continuity of care while supporting positive patient outcomes
Perform additional responsibilities as assigned to support patient care and branch operations
Graduate of an accredited nursing program
Current, unrestricted Registered Nurse (RN) license in the state of employment
Minimum of one (1) year of clinical nursing experience
Ability and willingness to travel throughout the assigned service area (approximately 50%)
Reliable transportation, valid driver's license, and current auto insurance meeting state requirements
Ability to lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment
Associate or bachelor's degree in nursing (ADN or BSN)
Previous Home Health or Hospice experience
Experience using Homecare Homebase (HCHB)
