Work Environment

The position is situated in an outpatient office setting within a skilled nursing facility, promoting a dynamic and interactive work atmosphere. You will work on-site, engaging closely with various members of the healthcare team including occupational therapists, nurses, and administrative staff. Key Responsibilities

• Conduct thorough examinations of patients to determine their needs and develop effective, evidence-based treatment plans tailored to achieve optimal recovery outcomes.
• Collaborate with interdisciplinary teams by attending care plan meetings, contributing insights on patient progression, and making recommendations for ongoing treatment approaches.
• Educate patients and their families about conditions, therapy processes, and home exercise programs, empowering them to take an active role in their rehabilitation journey.
• Document patients' progress through timely and accurate daily notes in compliance with legal and facility standards, ensuring that all records reflect each patient's treatment history and outcomes.
• Participate in facility in-service training to enhance understanding and utilization of therapy services among staff and caregivers.
• Maintain continuous communication with supervising Physical Therapists to ensure seamless continuity of care and foster an environment of shared knowledge.

Patient Population / Client Focus

You will primarily work with diverse patient populations, including elderly individuals recovering from surgeries, residents with chronic conditions seeking to enhance mobility, and those requiring post-injury rehabilitation. Your role will be crucial in addressing the unique challenges faced by each patient, ensuring they receive customized care that aligns with their personal health goals.

Qualifications

• A Bachelor's Degree in Physical Therapy or a related field is required. A Doctorate in Physical Therapy is preferred.
• Current, valid Physical Therapist license in Virginia in good standing.
• Current CPR certification.
• Possession of a valid driver's license and automobile insurance is necessary for community outreach and patient visits.
• Ability to demonstrate advanced critical thinking and problem-solving skills in the clinical environment.
• Strong interpersonal skills for effective communication with patients, their families, and team members.

Preferred Experience

• Prior experience in a Skilled Nursing Facility is desirable but not mandatory.
• Demonstrated success in developing and implementing rehabilitation programs that align with patient goals and improve therapeutic outcomes.

Compensation and Benefits
