Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to establish mental health diagnoses, develop individualized treatment plans, and implement short-term counseling strategies tailored to the needs of each patient.

• Utilize a variety of therapeutic modalities to address conditions such as depression, anxiety, trauma, grief and loss, addiction issues, domestic violence, and other psychosocial stressors.
\n
• Provide short-term case management services to support patients throughout their treatment process, facilitating resources and referrals as needed.
\n
• Collaborate effectively with team members to provide crisis intervention and triage services, including assessment and de-escalation strategies, and coordinating with external mental health crisis response teams when necessary.

• Engage in consistent follow-up assessments and prevention planning after crisis events, ensuring that all relevant agencies are involved and updated as necessary.

Patient Population / Client Focus\n

In your role, you will work closely with a diverse array of patients, including children, adolescents, and adults grappling with various mental health challenges. Your expertise will assist in addressing significant life stressors and fostering resilience in clients facing complex social or behavioral issues. Your commitment to understanding and accommodating the unique backgrounds and experiences of your clients will be paramount to their recovery journey.

Qualifications\n

• A Master’s Degree in Behavioral Science or related field.

• A minimum of 2 years post-licensure experience is required.
\n
• Current licensing as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).

• Demonstrated clinical skills in providing mental health services to individuals, families, and groups.
\n
• Profound knowledge of mental health disorders and the most effective treatment strategies tailored to individual needs.

• A valid driver’s license and regular access to reliable, insured transportation are mandatory for potential in-home visits.

Preferred Experience\n

• Experience with teletherapy and familiarity with digital systems for client care coordination and documentation.

• Two to three years of relevant clinical experience in a similar setting, including outpatient therapy, crisis intervention, or community mental health.
