Work Environment

This position is full-time and primarily onsite, focusing on delivering home-based care to patients. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians, utilizing evidence-based assessment techniques to determine their specific needs.
• Develop and implement individualized treatment plans that address each patient's unique conditions and goals, ensuring that interventions align with medical guidelines.
• Utilize various therapeutic modalities, including manual therapy, therapeutic exercises, and neuromuscular re-education, to facilitate recovery and enhance functional mobility.
• Document all patient interactions and progress in accordance with clinic protocols, ensuring compliance with HIPAA and other regulations.
• Maintain open communication with interdisciplinary teams and caregivers to discuss strategies for maximizing patient outcomes and address any challenges that arise.
• Deliver educational sessions to patients and their families, enhancing their understanding of their conditions and promoting self-management techniques.
• Participate in regular team meetings and continuing education opportunities to share knowledge, strategies, and best practices that improve patient care.

Patient Population / Client Focus

As a Home Care Physical Therapist at Sollis, you will work with a diverse patient population, including individuals recovering from orthopedic surgeries, managing chronic pain, and those needing rehabilitation post-injury. This role allows you to make a direct impact on patients' lives, particularly pediatric, neurologic, and sports medicine clients, by tailoring your approach to meet the intricacies of each case.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active license as a Doctor of Physical Therapy in New Jersey.
• At least two years of clinical experience in a physical therapy setting with demonstrable competencies.
• Current CPR certification.
• Strong verbal and written communication skills to effectively convey treatment plans and insights.
• Proficient in computer systems for documentation and reporting.

Preferred Experience

• Experience working in home health care or outpatient settings.
• Familiarity with innovative treatment methodologies and patient engagement practices.
