Key Responsibilities

• Conduct thorough evaluations of patients by reviewing historical data, performing assessments, and formulating tailored treatment plans based on individual patient needs.
• Effectively communicate evaluation findings and treatment strategies to patients and their families, ensuring that they are actively involved in goal-setting and decision-making for their rehabilitation journey.
• Implement evidence-based treatment protocols that address each patient's unique presentation and adjust treatment plans as necessary in response to patients' changing conditions.
• Collaborate with healthcare team members, including physicians and occupational therapists, to exchange insights on patient conditions and coordinate care effectively.
• Maintain accurate and timely documentation of patient care activities, including evaluations, treatment plans, progress notes, and discharge summaries, to ensure compliance with relevant healthcare regulations.
• Diligently manage time to meet productivity benchmarks while ensuring that all patient appointments and meetings are scheduled efficiently.
• Participate in department efforts to achieve established goals, contribute to community outreach programs, and remain compliant with accreditation standards.
• Pursue ongoing professional development through continuing education and mentorship, enriching both personal skills and organizational capabilities.
• Champion patient experience initiatives, providing adaptable care solutions to meet patients' needs and engage actively in efforts to eliminate barriers to access.

Patient Population / Client Focus

The ideal candidate will work with a variety of patient populations, addressing a spectrum of rehabilitation needs ranging from post-operative care to chronic pain management. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A current, valid state license to practice as a physical therapist in Virginia.

• Excellent communication and interpersonal skills, with the ability to engage effectively with diverse populations.

Preferred Experience

• At least two years of relevant clinical experience in a rehabilitation setting is strongly preferred, although new graduates are encouraged to apply.
