Work Environment

This onsite role focuses on providing home care to patients, meaning your work will take you directly to the living environments of those you serve. You will have the opportunity to work one-on-one with patients in the comfort of their homes, translating therapeutic exercises and rehabilitative techniques into practical, everyday contexts. Collaborating closely with an interdisciplinary team, you will create a supportive environment where patients feel safe and motivated to achieve their rehabilitation goals.

Key Responsibilities

• Implement therapy plans tailored to individual patient needs, emphasizing early intervention and same-day evaluations to enhance recovery timelines.
• Ensure effective clinical workflows, which optimize patient capture, manage referral processes, and coordinate scheduling efficiently.
• Engage in continuous clinical improvement initiatives, collaborating with the Director of Therapy Operations (DTO) to identify and implement best practices.
• Maintain precise and thorough documentation of patient care activities, leveraging clinical systems to ensure optimal coding, charge capture, and compliance with patient care orders.
• Foster relationships with employers, referral sources, and payer groups to enhance communication and collaboration on patient care strategies.
• Promote patient safety and high-quality outcomes by adhering to the guidelines outlined in the Physical Therapy Guidebook.
• Actively participate in case discussions, especially concerning patient outliers, to facilitate shared learning with staff therapists and improve clinical outcomes.
• Drive positive patient experiences and satisfaction metrics through thoughtful and empathetic interactions.

Patient Population / Client Focus

The primary focus of your role will be on individuals recovering from various conditions that may limit their physical capabilities. This may include post-surgical patients, individuals with chronic pain, or those dealing with neurological challenges. Your expertise will help these clients regain mobility, strength, and independence in familiar settings, promoting comfort and confidence as they navigate their recovery.

Qualifications

• Must possess a Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active licensure as a Physical Therapist in the state of Florida is required.
• Strong decision-making skills, with the ability to analyze problems, consider alternatives, and develop effective solutions logically.
• Demonstrated customer service excellence, with an ability to engage with patients and their families in a professional and respectful manner.

Preferred Experience

• Experience in home health care settings is highly desirable.
• Familiarity with various clinical software systems and electronic health record management.
• Previous involvement in quality improvement projects or clinical research within physical therapy.
