Work Environment

This position is primarily onsite, facilitating personalized home care therapies. The clinic promotes collaborative practice, fostering an atmosphere where physical therapists work alongside a diverse multidisciplinary team, including occupational therapists and ancillary staff, to coordinate comprehensive patient care. Key Responsibilities

• Conduct thorough patient assessments to understand their functional limitations and disabilities, utilizing evidence-based evaluation techniques.
• Develop and implement individualized treatment plans based on patient evaluations, adjusting protocols as needed to enhance therapeutic outcomes.
• Provide in-home therapy sessions, demonstrating the ability to adapt to the unique challenges that each home environment presents.
• Monitor and document patient progress meticulously, using electronic health record systems to assure accurate and comprehensive care documentation.
• Communicate effectively with patients and their families, providing education on treatment plans and exercises to facilitate ongoing treatment between sessions.
• Collaborate with healthcare professionals across disciplines to ensure cohesive patient care, recognizing when to refer patients for additional medical evaluation or specialty services.
• Actively participate in team meetings and continuous training programs to remain compliant with local, state, and federal regulations, ensuring the highest standards of care.

Patient Population / Client Focus

The Physical Therapist will primarily serve patients with diverse orthopedic conditions, physical disabilities, and post-operative needs within their homes. You will work predominantly with individuals recovering from surgeries, sports injuries, and chronic pain, tailoring interventions to optimize their recovery journeys and enable them to regain independence.

Qualifications

• A Doctorate of Physical Therapy (DPT) from a recognized and accredited physical therapy program.
• Current state licensure as a Physical Therapist in Virginia.
• Two (2) years of outpatient orthopedic experience, demonstrating competency in managing a variety of physical therapy cases.
• Current CPR certification, ensuring readiness for any emergency situations.

Preferred Experience

• Experience in home health therapy, with a preference for candidates who have managed patient care in various home settings.
• Familiarity with electronic health record systems and telehealth modalities.
• Interest in participating in clinical research or quality improvement initiatives.

Compensation and Benefits
