Key Responsibilities\n

• Conduct comprehensive assessments of clients' mental health needs and develop tailored treatment plans that align with their goals.

• Engage in regular communication with supervisors, healthcare teams, families, and other stakeholders to discuss clients' progress and adjust care strategies as needed.

• Organize and maintain a tentative schedule for client appointments, ensuring timely home visits and observations to facilitate in-depth evaluations and follow-ups.

• Coordinate data collection efforts, including chart reviews and staff interviews, to maintain an effective understanding of clients’ conditions.

• Ensure the effective implementation and adherence to treatment plans, participating in interdisciplinary team meetings to assess collaborative strategies and treatment outcomes.

• Document and complete progress notes on a monthly and quarterly basis to track improvements and provide updates to stakeholders.

• Lead training sessions that certify team members in the effective application of treatment plans and documentation procedures, thereby enhancing overall service delivery.

• Provide ongoing support to clients through follow-up sessions to assess needs and adjust approaches as necessary.

• Maintain accurate and thorough records that are accessible to authorized stakeholders, ensuring compliance with health information confidentiality standards.

• Act as an advocate and liaison for clients within the public mental health community to enhance program development goals.

Patient Population / Client Focus\n

Your primary focus will be working with a diverse population, including both adults and youth. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.
