Key Responsibilities\n

• Conduct thorough assessments of patients’ psychosocial needs to inform appropriate treatment plans and intervention strategies.

• Facilitate case management services by coordinating short-term counseling and crisis intervention based on each patient’s distinct circumstances.

• Assist patients in navigating complex systems by aiding them in completing necessary applications for entitlements, supplemental insurance, and other benefits that may enhance their access to care.

• Provide resources and support for patients seeking assistance with prescription costs, guiding them through indigent programs offered by pharmaceutical companies and community organizations.

• Advocate on behalf of patients when liaising with community agencies to ensure the provision of adequate support services.
\n
• Maintain meticulous documentation and patient records, preparing necessary reports and follow-ups to monitor treatment efficacy and compliance.

• Oversee and coordinate transportation assistance for patients who require it to ensure they can attend appointments and receive consistent care.

• Regularly attend team meetings and participate in clinical conferences to discuss patient progress, share insights, and refine treatment approaches.

Patient Population / Client Focus\n

As a Licensed Mental Health Counselor at Coastal Health Therapy Center, you will work closely with a diverse patient population, including children, adolescents, and adults dealing with various mental health issues such as anxiety, depression, trauma, and relationship challenges. Your role will involve developing tailored therapeutic plans that consider each patient's unique background and circumstances, fostering a supportive and trusting therapeutic alliance.

Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.

• Possession of at least one of the following independent licenses: Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two years of professional experience in the mental health field is essential to qualify for this role.

Compensation and Benefits\n
