Work Environment

This role is primarily onsite and entails providing home care services directly in patients' residences. Key Responsibilities

• Deliver daily physical therapy services in a home care environment, assessing each patient's progress toward their rehabilitation goals.
• Conduct thorough evaluations of patients, creating detailed documentation that adheres to organizational guidelines and healthcare regulations.
• Formulate and implement individualized treatment plans based on the patient's specific conditions and goals, utilizing clinically sound practices and rehabilitation techniques.
• Maintain accurate and timely documentation of patient progress notes, re-evaluations, and treatment sessions, ensuring compliance with standards of practice.
• Educate patients and their families on treatment options, rehabilitation exercises, and self-care techniques to enhance recovery outcomes.

Qualifications

Compensation and Benefits
