Work Environment

This position is fully remote, allowing for flexible work from the comfort of your own home. Our clinic utilizes innovative telehealth technologies to ensure seamless communication and quality care for our clients. You will have the benefit of working in a dynamic and nurturing environment, where your ideas and input will be highly valued. Typical work hours are Monday through Friday from 9:00 AM to 5:00 PM.

Key Responsibilities

• Act as a liaison between families and outpatient care providers to ensure cohesive treatment plans.
• Collaborate with insurance companies for pre-certification and utilization management processes, advocating for client needs.
• Engage in discharge planning and aftercare efforts to support clients transitioning from more intensive treatment settings.
• Provide therapeutic meal support and integrate nutrition education, fostering healthy relationships with food among clients.
• Deliver empathetic, safe, and affirming care, catering to the unique circumstances of each client while considering their diverse identities.

Patient Population / Client Focus

Our clinical focus encompasses a wide range of clients, including older adults facing mental health challenges, individuals experiencing trauma, and those coping with chronic illnesses. You will be tasked with tailoring your therapeutic approaches to address the specific needs of each client, ensuring that treatment plans are culturally and contextually appropriate. Your insights will help shape how we deliver services across our organization.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of New York.
• A foundational understanding and experience working with older adult populations.
• Proficient in clinical assessment and diagnostic skills, with the ability to develop comprehensive treatment plans.
• Exceptional interpersonal and communication skills, adept at building rapport with clients and collaborating with multidisciplinary teams.
• Ability to function autonomously while also contributing positively to team dynamics.

Preferred Experience

• Experience in a community health clinic or similar setting.
• Familiarity with telehealth modalities and remote patient management.
• Experience in conducting group therapy sessions or workshops.

Compensation and Benefits

This role offers a competitive salary ranging from $90,000 to $110,000 per year, commensurate with experience and qualifications. In addition, you will be eligible for a comprehensive benefits package that includes:
