Work Environment

This position is primarily onsite, as the Home Health Physical Therapist will travel to various homes within assigned areas in New Jersey to deliver one-on-one therapy sessions. We provide a supportive framework for our therapists, enabling them to focus on patient-centered care while working alongside other healthcare professionals in our outpatient facility for integrated care coordination.

Key Responsibilities

• Provide evidence-based orthopedic therapy tailored to each patient's unique needs directly in their home environments.
• Collaborate closely with our multidisciplinary team of in-house physicians, occupational therapists, chiropractors, and outpatient physical therapists to create cohesive care plans.
• Conduct thorough assessments of patients' safety, mobility, and functional capabilities in their home settings, adjusting treatment plans as necessary to meet individual goals.
• Educate patients and their families about exercises, body mechanics, and adaptive strategies aimed at enhancing recovery and maintaining optimal functional capabilities.
• Maintain accurate and comprehensive documentation of therapy sessions through secure and user-friendly electronic medical record (EMR) tools.
• Participate in interdisciplinary case reviews and engage in ongoing clinical education to stay current with best practices in physical therapy and rehabilitation.

Patient Population / Client Focus

Our patients include a diverse range of individuals recovering from major orthopedic surgeries, such as hip and knee replacements, as well as those with chronic conditions affecting their musculoskeletal system. Qualifications

• A current license to practice as a Physical Therapist in the State of New Jersey, or eligibility for licensure.

• Exceptional organizational skills to manage patient schedules and documentation efficiently.

Preferred Experience

• At least two years of experience in a homecare or outpatient orthopedic setting preferred, though confident new graduates are encouraged to apply.

Compensation and Benefits
