Key Responsibilities\n

As a Licensed Professional Counselor at St. Charles Counseling Services, your day-to-day responsibilities will include:

• Conducting comprehensive patient intakes, focusing on diagnostic evaluations and developing personalized treatment plans aligned with the patients' goals.

• Providing brief, focused therapeutic interventions for clients in need of mental health support, ensuring timely and effective care.

• Assessing patients for eligibility to remain in primary care settings versus referrals to specialty mental health facilities, guiding them effectively in their treatment journey.

• Monitoring patient progress through consistent follow-up, adjusting treatment plans as necessary based on individual needs and responses.

• Creating relapse prevention strategies and wellness plans to help patients achieve and maintain stability in their lives.

• Documenting all patient interactions, assessments, and treatment recommendations in comprehensive clinical records, while maintaining accurate progress notes.

• Educating patients on self-management techniques and promoting healthy lifestyle choices to empower them in their recovery process.

• Integrating behavioral health services and coordinating care between multiple providers, ensuring consistency in patient education and treatment adherence.

• Consulting with clinic management and multidisciplinary teams about behavioral services and improving program evaluations through evidenced-based practices.

Qualifications\n

The ideal candidate will possess the following credentials and skills:
