Key Responsibilities\n

• Participate actively in interdisciplinary care team huddles to align mental health strategies with overall patient care.

• Conduct thorough assessments and evaluations to develop individualized treatment plans tailored to each patient’s unique needs.

• Deliver both individual and group therapy sessions, utilizing evidence-based practices to promote emotional and psychological well-being.

• Consult with primary care medical providers regarding psychosocial issues that patients present during medical appointments, ensuring comprehensive treatment.

• Triage and manage urgent and routine referrals from medical providers, maintaining clear and efficient communication to optimize patient care.

Qualifications\n
