Key Responsibilities

As an integral part of our healthcare team, your primary responsibilities will include:

• Evaluating and treating patients recovering from injuries, illnesses, or surgeries in their home environment, ensuring that care is tailored to individual needs.
• Developing and implementing individualized treatment plans that incorporate various techniques such as exercise, hands-on therapy, and the use of rehabilitation equipment to alleviate pain and enhance mobility.
• Monitoring and documenting treatment progress, responses, and patient outcomes utilizing EMR software, including but not limited to Web PT, for accuracy and compliance.
• Adjusting therapeutic goals and creating new treatment plans based on ongoing evaluations to ensure continuous improvement in patient care.
• Providing training to patients and their families on the use of assistive devices and physical therapy techniques to promote independence and safety at home.
• Staying abreast of the latest research and advancements in physical therapy through educational workshops and professional development opportunities.

Patient Population / Client Focus

Your role within our organization will involve working with a varied patient demographic, including children, adults, and seniors, each presenting unique challenges and needs. You will be instrumental in assisting patients who have experienced conditions ranging from post-surgery recovery to chronic pain management, ensuring all clients receive personalized treatment to maximize their functional abilities.

Qualifications

To qualify for this position, candidates must possess:

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• An active Physical Therapist license in the state of Florida with no history of sanctions or board proceedings.
• Expertise in a range of manual therapy techniques and the ability to develop innovative exercise programs tailored to individual patients.

Preferred Experience

While not mandatory, candidates who have:

• Experience in home care environments will be given preference as this will enhance the ability to meet our patients in comfortable settings.

Compensation and Benefits
