Key Responsibilities\n

• Actively engage with clients to develop personalized care plans, ensuring proactive measures are in place to prevent crisis situations.

• Maintain up-to-date crisis alert documentation to effectively coordinate with county crisis services, ensuring timely and efficient responses to client needs.

• Facilitate transitions between hospital and outpatient services by coordinating with health care teams and documenting necessary information to support smooth discharges.

• Conduct regular follow-up sessions via telehealth, providing ongoing guidance and support tailored to the evolving needs of clients.

• Participate in interdisciplinary team meetings to discuss client progress and adjust care strategies as needed.

Qualifications\n

• A Master’s degree in Counseling, Social Work, Mental Health Counseling, or a related field.

• Must hold an active license as a Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

This role offers a competitive salary range of $100,000 to $105,000 annually, commensurate with experience and caseload.
