Work Environment

This position is performed onsite, primarily in the patient's home environment, fostering a high level of personal connection and tailored care. The therapist will interact directly with clients and their families, providing therapy in a comfortable and familiar setting while collaborating with an interdisciplinary team of healthcare professionals to ensure holistic patient care.

Key Responsibilities

The Physical Therapist will be responsible for:

• Implementing individualized treatment plans aimed at preventing disability, relieving pain, and restoring muscle function, while promoting maximum performance within each client's capabilities.
• Assessing client conditions utilizing diagnostic and prognostic tests related to muscles, nerves, joints, and functional abilities.
• Overseeing Physical Therapy Assistants to ensure they are implementing the care plan effectively and safely.
• Providing guidance on home modifications to promote rehabilitation and increase client independence.
• Administering therapeutic exercises, managing clients during non-manual exercises, and facilitating recovery through functional activities.
• Documenting and preparing clinical records, including evaluations, progress notes, and discharge summaries to maintain comprehensive patient files in accordance with applicable regulatory standards.
• Monitoring clients' progress and making necessary adjustments to their treatment plans while effectively communicating findings to physicians and case managers.
• Partaking in case conferences with interdisciplinary teams to evaluate treatment strategies and outcomes while offering professional insights influenced by patient needs and experiences.
• Educating clients and families about physical therapy protocols and the proper use of therapy equipment.
• Completing other administrative duties related to patient care and therapy services as assigned.

Qualifications

To be considered for this rewarding opportunity, applicants must possess:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid PT license in good standing in the state of New Jersey.
• Current CPR certification, a valid driver's license, and automobile insurance.
• One year of professional experience as a physical therapist, preferably in acute care or home care settings.
• A passion for clinical excellence, quality care, and a strong foundation in ethical practice.
• The ability to think creatively and embrace leadership within the workplace.
• A willingness to take responsibility for sound clinical decisions.

Preferred Experience

Experience working in home healthcare or hospice care environments will be highly regarded. Familiarity with electronic health record (EHR) systems and patient assessment tools will also be advantageous, as well as a background in interdisciplinary treatment planning.

Compensation and Benefits

Boulder Orthopedic Therapy Clinic offers a competitive salary range of USD 100,000 – 120,000 yearly, based on experience and qualifications.
