Physiocare Physical Therapy Clinic is seeking a dedicated and compassionate Physical Therapist to join our home care team in Lakewood, New Jersey. Key Responsibilities

• Conduct comprehensive Physical Therapy evaluations tailored for each patient, focusing on their specific diagnoses and rehabilitation needs.
• Develop, implement, and adapt individualized treatment plans, employing evidence-based practices to facilitate optimal recovery and progress.
• Regularly assess patient progress, utilizing standardized metrics and clinical observations to modify treatment plans as needed, ensuring alignment with recovery goals.
• Maintain thorough documentation including SOAP notes, progress reports, and care plans within the company's electronic health record system, ensuring compliance and facilitating seamless communication among care team members.
• Educate and empower patients and their families, providing guidance on recovery processes, expected challenges, and strategies for ongoing wellness.
• Achieve and maintain minimum utilization and productivity metrics as defined by clinic objectives, contributing to the overall operational goals of the organization.
• Collaborate with other healthcare professionals within the clinic to coordinate care, particularly for patients receiving multiple services, ensuring a holistic approach to treatment.

Patient Population / Client Focus

Our primary focus is to serve a diverse patient population, encompassing different ages, cultural backgrounds, and diagnoses. You will have the opportunity to work with pediatrics, geriatrics, and individuals with varying disabilities. Delivering care in the community setting allows you to develop meaningful relationships with patients and their families, thereby enhancing the rehabilitation experience and outcomes.

Qualifications

• A Master's or Doctorate in Physical Therapy from an accredited program.
• Current and unrestricted Physical Therapy licensure in New Jersey, in good standing with the Board of Physical Therapy.
• Willingness to perform evaluations and treatments both in-person and via telehealth, adapted to clinical appropriateness.
• A genuine enthusiasm for working with a broad spectrum of patients, embracing diversity in age, race, and ethnicity.

Preferred Experience

Compensation and Benefits
