Key Responsibilities\n

• Conduct virtual therapy sessions with individuals and groups, providing a safe environment for clients to express their emotions and share their experiences.

• Design and implement personalized, patient-centered treatment plans based on comprehensive assessments of emotional, social, and psychological needs.

• Collaborate closely with our interdisciplinary team, including physicians, nurses, and other mental health specialists, to integrate social work interventions into comprehensive care strategies.

• Maintain thorough and confidential client documentation in compliance with industry standards and legal requirements, ensuring accessibility for team members involved in the care process.

• Engage in regular supervision and continuous professional development to enhance skills and stay abreast of best practices in the hospice care space.

Patient Population / Client Focus\n

This position primarily serves individuals enrolled in hospice care, often facing end-of-life challenges. Clients may experience a range of psychological states, including anxiety, depression, and grief, necessitating a sensitive, understanding approach. Your interventions can help alleviate emotional distress, provide support during transitions, and facilitate discussions about advanced care planning. By utilizing culturally competent practices, you will help ensure that care is equitable and respectful of each client’s unique background and wishes.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• An active Licensed Independent Clinical Social Worker (LICSW) license in the state of Massachusetts is mandatory.

• A minimum of two years of post-licensure experience in a clinical setting, particularly with hospice or palliative care populations.
