Work Environment

This position is based onsite in clients' homes throughout the Chantilly area. The role requires flexibility, as you will travel to various locations to provide therapy services in a comfortable and encouraging environment. Key Responsibilities

• Direct and implement physical therapy treatment plans focused on relieving pain, restoring muscle function, preventing disability, and maximizing the patient's independence.
• Supervise and support physical therapy assistants in executing therapy protocols to ensure adherence to the established care plans.
• Conduct comprehensive evaluations of clients' physical abilities utilizing diagnostic tests to formulate accurate treatment strategies.
• Assess home environments for rehabilitation needs and make modifications to help clients achieve greater independence.
• Provide services as prescribed by physicians, including the administration of manual therapeutic exercises, motivational support, and functional activity guidance.
• Document and maintain clinical records, including initial evaluations, progress notes, and discharge summaries, ensuring adherence to OASIS requirements.
• Collaborate with the interdisciplinary team during case conferences, making informed contributions to the overall treatment planning process.
• Instruct clients and families on the use and care of home therapy devices and educate them on the entire therapy regimen.
• Complete functional assessments and revise care plans based on client needs and progress, ensuring comprehensive care delivery.
• Timely submission of orders and documentation following branch policies to facilitate smooth operational workflows.

Patient Population / Client Focus

Your primary focus will be on clients within the home care sector, including elderly patients recovering from surgery, individuals with chronic pain or injury, and those requiring rehabilitation from debilitating conditions. This role is pivotal in supporting their physical rehabilitation, encouraging patient engagement, and promoting overall wellness in a familiar environment that enhances their comfort and confidence during recovery.

Qualifications

• A Doctorate in Physical Therapy (DPT) with a focus on clinical practice.
• Active Physical Therapist licensure in the state of Virginia, ensuring compliance with all regulatory standards.
• Proven expertise in manual therapy techniques and a deep understanding of contemporary physical therapy practices.
• Strong analytical skills to assess and improve patient outcomes based on evidence-based practices.
• Proficiency in utilizing Electronic Medical Records (EMR) systems, especially Web PT, for documentation and clinical communication.

Preferred Experience

• Experience in home health care settings, showcasing adaptability to diverse patient needs and environments.
• Demonstrated effectiveness in designing and implementing customized exercise programs that drive patient rehabilitation.
• Knowledge of the latest innovations in physical therapy methodologies and technologies.

Compensation and Benefits

The salary for this role ranges between USD 100,000 and 120,000 per year, accompanied by a comprehensive benefits package.
