Key Responsibilities\n

• Conduct thorough ongoing assessments to understand and address the unique needs of each client, which may involve structured interviews and standardized screening tools.

• Facilitate effective communication between supervisors, healthcare team members, and family members, ensuring a holistic approach to client care and regular updates on progress.

• Develop and provide a tentative schedule of appointments, incorporating home visits and observational assessments to closely monitor client engagement and therapeutic advancement.

• Coordinate home visits in compliance with therapeutic goals, documenting observations and engaging with clients in their natural environments.

• Ensure that the implementation of the treatment program plan is not just effective, but responsive to changing client needs and feedback.

• Actively participate in Interdisciplinary Team Meetings, contributing valuable insights regarding client progress and treatment updates.
\n
• Document and complete comprehensive monthly and quarterly progress notes, maintaining meticulous records for continuity of care.

• Organize and facilitate training sessions for staff to ensure competency in the implementation of the program plan and best practices in documentation.

• Provide ongoing follow-up support through interviews and home visits, ensuring that clients continue to have the assistance they require.

• Maintain accurate records so that essential information is accessible to authorized personnel and involved parties.

• Serve as a liaison with the public and the broader mental health community, facilitating partnerships that enhance support and resource availability.
