Key Responsibilities\n

As a Licensed Professional Counselor with Medic Care & Health Services, your day-to-day responsibilities will include:

• Conducting comprehensive assessments to identify the mental health needs of clients, crafting tailored treatment plans accordingly.

• Maintaining clear and ongoing communication with supervisors, direct care staff, and clients’ families to provide updates on individual progress and adjust care strategies as needed.

• Organizing and scheduling home visits and appointments, ensuring adherence to the outlined therapeutic strategy and client engagement.

• Facilitating interdisciplinary team meetings to discuss client cases and contribute valuable insights for effective program implementation.

• Tracking and documenting client progress through monthly and quarterly progress notes, ensuring all records are meticulously maintained and accessible.

• Training and certifying other staff members on program changes and documentation, promoting an environment of continuous learning.

• Acting as a liaison with the public and other mental health organizations to advocate for clients and community resources.

Qualifications\n

Applicants must possess the following qualifications to be considered:
