Key Responsibilities\n

• Conduct detailed psychosocial assessments of patients to identify unique psychosocial needs, strengths, and challenges, ensuring every aspect of their care is addressed.

• Develop individualized care plans in collaboration with patients and an interdisciplinary healthcare team, focusing on behavior modification, coping strategies, and sustainable lifestyle changes.

• Ensure accurate and efficient documentation of patient interactions, progress notes, and care plans in our electronic health record (EHR) system, thereby maintaining compliance with documentation standards.

Qualifications\n
